Clinical programs and pricing
Most women begin with Know Your Number when they want clarity about what is changing. From there, the Metabolic Healthspan Shift gives guided, 12-week support to act on those numbers, and the Metabolic Healthspan Reset gives 16 weeks of active, clinician-led care. What comes next follows from your own numbers.
Individualized medical care is available by telehealth to women physically located in AZ, CO, FL, ID, KY, MT, OR, WA, and WY.
Where to begin
Either option credits toward the Reset if you continue within 30 days.
You know something has changed, but you are not sure what deserves your attention first. Know Your Number gives you a clearer picture of your metabolic health, your estimated biological age, and the two or three areas most likely to move your health forward. You leave knowing whether a focused program fits, or whether lighter care is enough.
Your Healthspan Snapshot
Know Your Number gives you your Healthspan Snapshot. We take your own bloodwork, your Healthspan Core Panel, read each result against the optimal ranges used in my practice, alongside the laboratory’s standard reference range, and turn it into a clear picture: where you are now, which patterns stand out, and the two or three priorities that most deserve your attention first. You leave knowing exactly where you stand and what matters most. The Reset goes deeper on its own, with a broader laboratory panel, including a more complete hormone and thyroid evaluation, that becomes your full personalized Metabolic Map, with a week-16 retest that shows what changed. You can begin with the Reset directly. Starting with Know Your Number is optional, and the Reset includes its own baseline.
Guided middle-tier care
A guided, 12-week hybrid program for the woman who is ready to act on her numbers, improve her metabolic health, and build stronger habits, with a clear plan and structured support, without the seven-visit intensity of the full Metabolic Healthspan Reset.
The Shift is 12 weeks, clinician-designed with support at key checkpoints. The Reset is 16 weeks, clinician-led with more frequent visits and an expanded lab panel. Both begin from your own numbers.
$1,200 total. Pay in full, or three monthly payments of $400, first payment at booking.
For the woman who is ready to act on her numbers with a clear plan and structured support between a handful of focused visits, rather than the full clinician-led intensity of the Reset.
Available to residents of AZ, CO, FL, ID, KY, MT, OR, WA, and WY. You must be physically located in a licensed state at the time of your visit.
$1,200 total, or three monthly payments of $400. First payment at booking.
Focused, active care
When you are ready for active, structured change, the Metabolic Healthspan Reset brings your laboratory patterns, estimated biological age, nutrition, muscle, hormones, thyroid, and metabolic health together in one individualized 16-week plan. The program adapts to your situation, including whether you are taking a GLP-1, preparing to transition off, already off one, or working without one.
More energy. Greater strength. A metabolism that feels less like a constant fight. A clearer understanding of what is driving the changes in your body, and a plan you can actually follow.
Over 16 weeks, we use your own numbers to decide where to focus, make targeted changes, and then remeasure key markers so we can see what changed and what still needs attention.
Your plan is built from your labs, health history, symptoms, goals, and response to treatment, rather than a generic diet, standard supplement bundle, or the same protocol handed to every woman.
The work reaches beyond weight, toward the metabolic health, muscle, strength, and resilience that help you stay capable for the years ahead.
What your Metabolic Map is
Your Metabolic Map starts with your own comprehensive bloodwork, more than 90 markers, each read against the optimal ranges used in my practice, the levels associated with feeling and functioning well, alongside the laboratory’s standard reference range. Reading against optimal lets us see the patterns that are beginning to drift while they are still early and changeable, rather than waiting until a result has already crossed into abnormal.
From there, your Map becomes your plan. Your own personalized report shows where your results fall against those ranges, the patterns and drivers that stand out, the areas that are already working well, and the areas that deserve attention, then connects them to your food, protein, and targeted-supplement recommendations and the priorities that shape your metabolic and healthspan plan, built around your own numbers rather than a one-size bundle. The goal is to identify important changes early, before they become a more significant health problem, and when a diagnosis is already present, to reduce its impact on your health today and your risk in the years ahead. At week 16 we retest and rebuild your Map so you can see what changed, what improved, and what still deserves attention next.
Pay in full or choose four monthly payments of $800.
Move beyond another generic reset.
The Reset is for the woman who wants meaningful metabolic change, whether she is staying on a GLP-1, preparing for a transition, already off one, or working without medication. It is built around your own bloodwork and adjusted to where you are.
The week-16 retest shows what changed, what did not, and what deserves attention next.
If you completed Know Your Number within the previous 30 days, your fee applies toward the program.
Built around your situation
Your GLP-1 status does not send you to a different program. It changes how the Reset is individualized: the nutrition, protein, muscle work, laboratory monitoring, and timing all adapt to where you are. If you keep an outside prescriber, that clinician remains responsible for the GLP-1 medication. If Root & Remedy prescribes it, Sheri Erwin, DNP, APRN, FNP-C, manages the medication as part of your care.
The Reset helps you protect muscle, strength, nutrition, and metabolic health during weight loss, with monitoring and support matched to your situation. If you keep an outside prescriber, that clinician manages the medication; if Root & Remedy prescribes it, Sheri Erwin, DNP, APRN, FNP-C, manages it as part of your care.
We build the nutrition, protein, muscle, and metabolic foundations before and during a transition, so your progress holds instead of unraveling afterward.
The Reset helps you understand and respond to weight regain or other metabolic changes after medication, using your laboratory patterns, nutrition, strength, and health history to guide the plan.
Real metabolic change from your own physiology, no medication required, built around your bloodwork and your Metabolic Map.
After your focused program
At the end of a focused program, or after Know Your Number or another appropriate assessment, Dr. Sheri will recommend the level of continuing care that fits your progress, clinical needs, and the support you want. These memberships are for established Root & Remedy Integrative Care patients when clinically appropriate. Neither is a lower-cost substitute for the Reset. Ongoing Metabolic Care is focused metabolic follow-up; the Comprehensive Healthspan Membership is the broader, longitudinal relationship where ongoing hormone and thyroid management lives alongside whole-health clinical care.
Keep your progress from quietly slipping.
Focused, clinician-guided metabolic follow-up for established patients who want continued help with nutrition, strength, laboratory follow-up, and progress. It may be recommended after Know Your Number when lighter clinical support is enough, or after the Reset.
One clinician to connect what feels disconnected.
The longitudinal clinical relationship for women who want one clinician looking across their whole health over time. This is where ongoing hormone and thyroid management lives, alongside metabolic care, cardiometabolic risk reduction, bone and muscle health, prevention, medication management, follow-up testing, gut concerns when relevant, and long-term healthspan planning. It can begin after the Reset or after another comprehensive assessment with Dr. Sheri when it is the right clinical fit. It is a standing clinical relationship, not maintenance. Personalized meal planning is included.

Separate specialized care
Cancer risk reduction and early detection, from a clinician who spent years in oncology. We map your personal and family risk and build a screening plan matched to you. This is a separate specialized prevention service, not part of the metabolic pathway. $695.

Supportive therapies and add-ons
Optional, clinician-guided support alongside your care: NAD+, Lipo-C, select peptides, and practitioner-grade supplements, including support for women staying on, coming off, or working without a GLP-1. These are add-ons to a plan, never a substitute for it.
