Notice of Privacy Practices: how Root & Remedy Integrative Care may use and disclose your protected health information, and your rights.
We are legally required to provide notice regarding how we may use and disclose your protected health information. We reserve the right to change these policies at any time, with notice to you. If a policy is changed, it will apply to all of your current and past health information. You may request a copy by contacting our office at 2519 S. Treatment. Your information may be used to provide care and to coordinate with other providers and staff involved in your care.
Payment. Your information may be shared with payment processors as needed to process your payment.
Health care operations. Uses include staff training, case review, quality improvement, and appointment reminders by phone, email, or text.
Business associates. When we share information with a business associate, we require a written contract with terms that protect the privacy of your information.
Marketing. You may receive promotional materials, and you may opt out at any time.
Authorization. We will not use or disclose your protected health information for any purpose other than those identified here without your specific, written authorization.
Family and friends. Information may be shared with family or friends present during your care if you do not object.
Other permitted disclosures. Your information may be disclosed as permitted or required by law, including for research (with authorization), organ donation, public health and safety, health oversight activities, workers' compensation, court orders and subpoenas, and lawful law-enforcement requests.
Access. You may access and request copies of your protected health information in writing. We may charge a reasonable fee for the time to obtain and copy it.
Amendment. You may request corrections to inaccurate or incomplete information in writing, with an explanation. Denials will be explained in writing.
Accounting of disclosures. You may request a list of certain disclosures, excluding those for treatment, payment, and operations, for up to six years prior to your request.
Restrictions. You may request in writing that we limit how your information is used or disclosed, except where we are legally required to disclose it.
Confidential communication. You may request that we communicate with you by a specific method or at a specific location.
Paper copy. You may request a paper copy of this notice at any time.
Complaints. You may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
The following applies to information collected through the public website, including the Find My Supplements wellness quiz. This is draft language pending attorney review. Health information you provide through a public wellness quiz is separate from the protected health information handled under the clinical notice above.
If you complete a wellness quiz, health assessment, or supplement questionnaire, we may collect information you voluntarily provide about your wellness goals, medication categories, health conditions, pregnancy status, supplement use, symptoms, or similar information relevant to generating your results. We use it to provide quiz results, generate supplement and wellness suggestions, identify situations where additional safety guidance may be appropriate, respond to you, and improve our website and educational services. We seek to collect only information reasonably necessary for these purposes, and we ask about medication and condition categories rather than your full medical history. Completing a public wellness quiz is not a substitute for information provided through the clinical patient portal, and where required by law we obtain your consent before collecting or processing this information.
We may use third-party service providers to operate portions of our website, quizzes, email communications, and other business functions. Information submitted through these services may be processed by those providers on our behalf under applicable agreements. We do not sell the health information you provide through the quiz, and we do not use it for targeted advertising.
